Breath Rates & Ratios
Today's ACLS tip is on rescue breathing and ventilation rates & ratios used with and without an advanced airway while doing CPR. How often to give a breath while administering rescue breathing to someone with a pulse. Advantages of using end tidal CO2 wave form capnography. Giving breaths while CPR is in progress prior to an advanced airway being placed. Types of advanced airways and when they should be used during a code. Giving breaths while CPR is in progress and placement of an advanced airway has been confirmed. ETCO2 to: confirm placement; monitor effective CPR; and identify ROSC. Delivering Uninterrupted Chest Compressions
Minimizing interruptions to chest compressions to under 10 seconds so that we can maintain a chest compression fraction of at least 80% has been shown to improve patient outcomes. Continuous chest compressions helps maintain coronary perfusion pressure needed to deliver oxygenated blood to the heart. End tidal CO2 and real time feedback should be used to measure effective CPR. Pauses to chest compressions should be timed and kept to under 10 seconds. Chest compressions may be continued while the defibrillator charges and during insertion of advanced airways. We should coordinate activities that are performed during compression pauses to minimize interruptions. Once an advanced airway is in, we don't stop chest compressions for breaths. We simply give one breath every 6 seconds. If a team member identifies that chest compressions have been stopped for 10 seconds they should speak up.
