In this, our first EVER episode we look at the size of the problem. We look into the dark corners, at workarounds, at how the system can either raise you up or pull you down. We look at every part of the medication chain (well maybe not every part...) We'll give some scary numbers, tell tales of data and personal experiences of medication error, and think about the second victim.
