On Episode Three of the Plastic Surgeon Podcast, Dr. Sajan sits down with a fellow physician, Dr. Alexandra Kohles to discuss her experiences with plastic surgery. Nearly twenty years ago, Dr. Kohles underwent a breast augmentation in Chile where she got textured silicone implants. At the time, these breast implants were the most modern implant. These implants are no longer on the market.
After experiencing hardness in her breast, Dr. Kohles realized she needed breast revision surgery. She came to Dr. Sajan where he removed her implants and corrected capsular contracture. He then placed new implants under the muscle.
Along with the story of her breast augmentation and revision surgeries, Dr. Kohles discusses taking control of femininity with plastic surgery, capsular contracture, and finding confidence with breast implants. Dr. Kohles offers a unique perspective on breast augmentation and plastic surgery as a doctor.

Breast Augmentation With Different Sized Implants

On this episode of the Plastic Surgeon Podcast Dr. Sajan’s patient, Koreen. Koreen underwent a gummy bear breast augmentation for larger breasts and to treat breast asymmetry. She opted for two different size implants to achieve the most symmetrical results that matched her body.
Koreen is an ICU nurse who grew up all around the United States due to her father’s military career. Both of her parents immigrated from the Philippines and instilled values of positivity and hard work in Koreen. Becoming a nurse was natural for her given her childhood and personality.
Interested in undergoing breast augmentation surgery, she consulted with several surgeons who Koreen felt were selling themselves and not valuing what she wanted to achieve with the surgery. During her consultation with Dr. Sajan, Koreen knew she had found the right surgeon when he thoroughly discussed his plan based on her goals and anatomy and not the other way around.

I Was Meant To Be A Boy

On this episode of the Plastic Surgeon Podcast Dr. Sajan’s patient, Liam. Liam is a transgender man who underwent top surgery with Dr. Sajan at Allure Esthetic earlier this year. Liam was born and spent his early life in the Dominican Republic. It was not until he attended high school in the United States that he began to have the language for his identity and the feelings he had growing up.
During childhood, Liam experienced feelings of gender dysphoria. He never liked wearing traditional girls’ clothing and expressed an interest in girls instead of boys early on. Seeing boys, Liam realized that he wanted to look like and be like them. He even enjoyed when he would be misgendered before he transitioned because it better matched his true identity.
After moving to Washington and attending school, Liam witnessed fellow students come out as transgender and transition. This gave him the language to understand how he was feeling and his true gender identity. Liam did not formally come out until 2020, despite having these feelings all of his life.

LDS Conversion Therapy To FTM Surgery

On this episode of the Plastic Surgeon Podcast Dr. Sajan’s patient, Oaklee Gose. Oaklee is an FTM man who underwent top surgery with Dr. Sajan at Allure Esthetic. Growing up in Idaho and the LDS church, Oaklee’s journey is truly unique and captivating. It includes periods of homelessness, conversion therapy, and a lot of hard work.
Oaklee was an accomplished soccer player throughout childhood and got kicked off the team when he came out as a lesbian. This also caused his parents to kick him out of the house and eventually send him to conversion therapy. When he was a teenager and early adult, he learned more about binding and the transgender community from a friend and the internet.
After moving to Oregon on a soccer scholarship, Oaklee began to identify as transgender. This is when he looked into hormones and top surgery. While attending school to become a paramedic, Oaklee was able to undergo top surgery with Dr. Sajan. Taking hormones and top surgery completely changed his life, and according to Oaklee, saved his life.

Transgender Discrimination Within The Community

Joining Dr. Sajan on the Plastic Surgeon Podcast is his patient, Ellis Younts. Ellis underwent bilateral double incision chest reconstruction, also known as FTM top surgery. He discusses the many decisions that transgender patients, doctors, and surgeons must make when considering transition. Talking points include underage gender affirming surgery, puberty blockers, and starting hormones at a young age.
Ellis provides important insight into the politics that exist within and outside of the transgender community concerning transition. He experienced gatekeeping being told he could not present feminine at all to consider himself a transman. Along with this, Ellis discusses early experiences in his life that foreshadowed his future gender identity.
With interesting and insightful conversations about what age gender is conceptualized, patient’s right to choose, and the possibility of regretting surgery. Luckily, Ellis does not regret his surgery and is so happy with his new chest following FTM top surgery.

Runaway Patient, Doctor Bashing & 1100cc Implant Intervention

Episode Sixteen of the Plastic Surgeon Podcast with Dr. Javad Sajan and Sabrina Sajan, covers the story of a runaway lip filler patient who wandered around the office with numbing cream on their lips. She was anxiously looking for their provider who was held up due to another patient who tried to bring their pet into the office.
Next, Dr. Sajan tells an unbelievable story about a fellow doctor who talked bad about Dr. Sajan for seeing a patient who believed she was suffering from breast implant illness. The other doctor had placed the breast implants and did not believe that breast implant illness exists. Not only does Dr. Sajan disagree, but he performs many en bloc breast implant removal surgeries to treat breast implant illness.
Finally, Dr. Sajan tells the unfortunate story of a patient who had breast surgery in Mexico that caused her breast implant to pop out during a routine mammogram. Dr. Sajan has worked with this patient over several months to resolve the issue and get her the breasts of her dreams.

Breast Augmentation Confidence from Childhood Abuse

Coming on the Plastic Surgeon Podcast this week is Dr. Sajan’s patient, Mercedes Christion. Mercedes underwent breast augmentation surgery with Dr. Sajan and was one of the patients featured in his TV series, realdrseattle®.
Mercedes always had some negative feelings and self confidence issues concerning her chest and its size. She found herself comparing herself to other women in her life with larger chests and considered breast augmentation from a fairly early age. Eventually, after suffering multiple work injuries that placed her on disability and caused her to suffer from muscle atrophy, the size of her chest was even more noticeable. This is when Mercedes began her research into plastic surgeons.
Mercedes followed Dr. Sajan on social media and was impressed with his many breast augmentation before and after photos. They were exactly the results she wanted. When she met with Dr. Sajan for a consultation, she was also glad that he did not encourage her to go bigger and respected her wishes to stay a natural size.
Mercedes’ story is one of triumph over childhood abuse, lack of confidence, and disability. Tune in to hear about how Mercedes prevailed over these obstacles to become the woman she is today.

Facial Feminization Surgery In Rural America

On this episode of the Plastic Surgeon Podcast Dr. Sajan’s patient, Kelli tells her story of growing up in rural Montana, coming out as a trans woman, and the difficulties and beauty in her transition. Kelli underwent facial feminization surgery with Dr. Sajan.
Kelli grew up in a small town of just 111 people in Montana. From the age of 5, she always identified more with her sisters and a feminine identity. Privately, her family embraced this, but publicly, she had to continue to present as a boy.
In high school, Kelli came out as bisexual and gay as a way to present more femininely. This cost her a sports career and led to bullying and mental health struggles. By college though, Kelli learned about transgenderism and began to openly identify as a trans woman.
Kelli’s surgery was unique. Many other surgeons she consulted with did not want to do the procedure as she wanted it. After coming to Dr. Sajan who helped her customize her FFS procedure, Kelli knew he was the right surgeon. She loves her results and could not be happier with her new face and the outlook it has given her.
