As an LNC, you may have encountered the medical/legal issues that arise when a hospital’s chain of command and also nursing administration isn’t clearly defined. These problems can especially affect a nurse who sees a problem with a medical or pharmaceutical order or with a patient’s condition and is unable to get anyone to address the situation. It’s a maze-like institution, and it can be very difficult to navigate. For this reason, many people hire lawyers to help them navigate the system. Most legal cases concern Social Supplement Income (SSI), which may apply to those not yet old enough for Social Security. Those in government are wrong who claim that huge fraud rates occur in SSI applications. Adriana de la Torre is an attorney who works in the area of Social Security claims. As a patient, family member, or nurse, you may have visited an ICU. Now you have the opportunity to experience the drama of this environment as described by a nurse who works there. Amy McCutcheon, who is both an LNC and a critical care nurse, recounts some of the conditions and circumstances that make the ICU unique among hospital settings. In chaotic and intense circumstances, all medical personnel must learn to work cooperatively. The ready availability of doctors and pharmacists makes the work easier. However, all those who work in ICUs suffer what is called alarm fatigue. In addition to the high noise level, a nurse must be able to discern which one needs to be handled first. Though nurses in ICUs have fewer patients, charting is much more complex and can’t be put off. Daily rounds, too, are more complicated, and involve many more people. Of particular importance to LCNs are errors in heparin and insulin titrating drips and the tendencies of overworked personnel to copy and paste reports from previous days, often without changing or noting the date. Anyone who’s had close contact with a newborn baby thinks they know how fragile they can be. Joan Earley, neonatal nurse for 30 years, who is also an LNC, will take you deeper into the intricate world of neonatal medicine, a world in which high-level communication, coordination of functions, a well-established chain of command, and split-second decisions can mean the difference between life and death. Added to these responsibilities is the difficult talk of telling parents that their infant has medical problems. This requires great compassion, and the continued need to update the parents about the baby’s condition calls for patience and clear explanations. The challenge of neonatal medicine begins before the baby is born in the cases of premature babies. Resuscitation can be a critical issue, and respiration monitoring is vital for all premature babies. In terms of liability, a large number of issues can result from the birth process itself. Another area of concern is skin breakdown. A premature baby’s skin condition must be frequently monitored. They lack the thickness of older baby’s skin, and, in addition, they frequently get jabbed, hooked up to IVs, and suffer other practices that violate the skin’s integrity. They are thus likely to suffer skin deterioration.
