Every aspect of a health center’s operations can be tracked using data, but it is the health centers that utilize data to problem-solve and innovate that are the most successful. Not only does this podcast discuss the ways data can be tracked and what should be tracked, but also methods to apply data to innovation to avoid its collection being “busy work.”

Data expert and former Coleman Associate Carlos Avina, joins us in this podcast to discuss the ways health centers can utilize data creatively. In our order for data to be used to its potential, you must develop it, understand it, and tie it back to a purpose. This process begins by identifying goals and data metrics that relate to those goals which provide the opportunity to assess yourself against industry standards.

According to Avina, this process of data utilization can help an organization identify problems, create solutions to pre-existing problems, and be predictive of patterns within the organization. Through healthcare and other examples, Avina will navigate you through starting the big data conversation within your organization to ways to fully utilize data.

This weekend we recorded a special episode in relation to the current events. We are going to explore how habits can diminish our ability to combat COVID19 in the healthcare setting. We interview Mellisa Stratman and explore some strategies around communication, scheduling, decision making, and coaching. We hope you enjoy it, and please be safe out there.
Synopsis: We are in unprecedented territory with COVID-19, and it is crucial to remain innovative in our ability to combat it in the healthcare setting. This podcast offers insight into how our habits can help or diminish our ability to do this. Through discussing strategies around communication, scheduling decision-making, and coaching, we want you to be better equipped to combat COVID-19 in a healthcare setting.

When communicating, make it a habit to relay to people what is in it for them, and connect the communication directly to the person. Joining behaviors and outcomes can alter people’s perception of communication and will help interest them in hearing what message follows. Additionally, your discussions must make people feel supported and acknowledged. Lastly, always make a habit of communicating clearly.

After communication, this podcast evaluations scheduling decisions. Does your patient need to come in for their follow-up, or can their needs be met through a virtual visit? The habit is meeting face to face, but by making this decision with patients, we can change that habit and prioritize in-person access for groups that need it most.

Lastly, this podcast offers insight on how to coach to be successful. With the vast amount of change happening right now, staff must be coached through these changes. Once you decide to do a new process, communicate well, guide your team through the transition by leading through example, and giving feedback to ensure improvement and success

In the old days, everything was on the doctor, and in those days, it was mostly physicians. Doctors did house calls, in-office care, hospital care, and everything else.
Those doctors would probably look at today's healthcare system with awe and amazement and a decent amount of head-spinning. That's because our understanding of the human brain and body and our focus on prevention has expanded greatly. There is now so much more to do for any given patient that it's nearly impossible for one person to take care of it alone. Any healthcare worker knows this just by looking at their overflowing inbox.
The obvious solution to this conundrum is team-based care. But, the term team-based care is thrown around so flippantly in healthcare that it has lost much of its meaning. Building strong patient care teams is a critical step in preparing for the future of healthcare, decreasing burnout, and improving patient outcomes.
The Sheep and Shepherd Model turns the hierarchy intrinsic to healthcare on its head and systematizes the team-based care concept. But if it were that easy....everyone would already be doing it.

In this special Leadership Series, we interview Amy Feimer, CEO of Hunter Health, a FQHC and Urban Indian Health Program in Wichita, Kansas. We learn how Amy and her team achieved a 36% reduction in their patient cycle time (70 to 45 mins), a 99% charting completion rate, a 48% reduction in their abandoned call rate (25% to 13%) and an increased staff engagement score in the 89% percentile.
This episode highlights ways to improve both provider and staff satisfaction in ambulatory care. Amy describes the ways in which her organization improved upon their abandoned call rate, their cycle time, and their documentation process and turn around time. Additionally, we hear insights she has had on how to lead people through change and the ways in which coaching people on data can be beneficial.

Hunter Health reduced their abandoned call rate by redefining roles responsible for that rate and by using data to anticipate when peaks would require staffing shifts. Cycle time reduced nearly 35 minutes by implementing the DPITM team dance workflow and a sheep and shepherd model of care. In this model and workflow, there is a designated lead in charge of directing workflow. This model also improved upon their documentation process as teams began charting in real-time.

Through these changes and the implementation of a validation process, the organization has jumped to the 80th percentile in staff engagement scores. Listen to this podcast to hear exactly how Hunter Health was able to implement these changes, as well as how it helped their staff.
