Optimal management of bipolar disorder begins with accurate diagnosis. The Depression and Bipolar Alliance reported that the misdiagnosis rate is 67% for bipolar patients with the most common misdiagnoses being unipolar depression (60%), anxiety disorder (26%), schizophrenia (18%), and borderline or antisocial personality disorder (17%). When diagnosis is inaccurate or delayed, initial therapy is not properly derived or promptly delivered. In this series of neuroscienceCME Chart Reviews, the faculty will take participants through a series of patient cases focusing on assessment, treatment, and treatment adherence to improve the long-term management of patients with bipolar disorder. Faculty will engage participants in the discussion and development of best practice tips to apply in practice.
Attention-deficit hyperactivity disorder (ADHD) affects 8% of school age children. It presents with symptoms of inattention, hyperactivity/impulsivity, or both. Currently, three subtypes of ADHD are defined: ADHD inattentive (ADHD/I), ADHD hyperactive/impulsive (ADHD/H) and ADHD combined (ADHD/C). However, in the last decade heated debate has emerged that questions how this disorder should be conceptualized, defined, and categorized. Key questions have been: Is it a category or a continuum? and How do comorbidities relate to currently defined subtypes? In these interactive, evidence-based neuroscienceCME Journal Club sessions, the faculty will explore new data that may re-focus how we view ADHD subtypes. Such exploration will allow clinicians to anticipate how developers of the DSM-V might revise ADHD diagnostic criteria. Until the past decade, there was generally a pessimistic attitude about the treatment of patients with schizophrenia and their outcomes. One key concept that has been identified as critical in optimizing the care and outcomes of patients is continuity of care by a team of healthcare providers. Continuity of care is widely viewed as a key quality indicator for outpatient mental health care and is essential to prevention of relapse and re-hospitalization. Yet, progress has lagged in the implementation and measurement of continuity of care in clinical practice. In a recent educational activity by CME Outfitters, over 220 psychiatrists responded that their main reason for not changing practice immediately regarding continuity of care was they did not know where to start, demonstrating a gap in knowledge and performance regarding the implementation of this process in clinical practice. A first step to improving care is identifying the barriers that exist. Patient barriers, physician barriers and system barriers can be significant obstacles requiring education of the entire mental healthcare team in order to begin to break down the walls and allow clinicians to provide the best care for each patient. The identification of the barriers can be the first step in empowering both the provider and the patient. This can highlight to psychiatrists, other healthcare providers, payors, and healthcare systems what barriers may exist and how they are impacting the outcomes and lives of patients. In this neuroscienceCME Live and On Demand activity, faculty will highlight barriers to optimal care and their impact on the outcomes and lives of patients with schizophrenia and propose practical strategies and tools that can be utilized to improve the care of patients with schizophrenia. One of the common misconceptions about ADHD is that it occurs only in children. The reality is that ADHD can affect people of all ages. It is estimated that in the United States alone, nearly 8 million adults have ADHD. While inattentiveness, impulsivity, and hyperactivity are the same hallmark features of both child and adult ADHD, these symptoms often manifest quite differently in adults. For example, hyperactivity in the child may be excessive running and climbing, while in the adult it is driving too fast. Impulsivity in the child can show up as blurting out answers in class, whereas for the adult interrupting colleagues during a business meeting would be likely. Recognition of the symptoms of ADHD that impact everyday life—at home, at work, and socially—is critical. The good news is that adult ADHD is a treatable medical condition, when recognized and addressed with an eye toward optimal management. Current data suggests that further continuing education and professional development is warranted to address clinical practice gaps related to diagnosis and management of ADHD in adults. Studies have shown that children with ADHD have higher rates of academic difficulties, social issues, and a number of comorbid psychiatric conditions compared to children without ADHD. These factors, and others, contribute to the high cost of care as well as diminished quality of life for children with ADHD. Applying the diagnostic criteria and unraveling the complexities of real-world patients can present a serious challenge to all clinicians. The symptom domains of ADHD—hyperactivity, impulsivity, and inattention—must often be diagnosed in the context of psychiatric comorbidities. The Multimodal Treatment Study of Children with ADHD (MTA) found that in a sample of 579 children, 31% had ADHD alone, 21% had oppositional symptoms, 12% had oppositional symptoms plus anxiety, 10% had anxiety symptoms, and 7% had comorbid conduct disorders. Careful clinical assessment of the child focusing on family history and longitudinal course of the disease is pivotal in order to develop appropriate treatment planning. A multimodal treatment approach that includes both pharmacologic and behavioral interventions is prudent. Ongoing monitoring at regular intervals utilizing structured data tools should be implemented to evaluate efficacy, tolerability, and compliance. In this evidence-based neuroscienceCME Live and On Demand activity, the expert faculty will explore the complex presentations of ADHD and provide strategies for the assessment, diagnosis, and individualized multimodal management of children and adolescents with ADHD. Multiple sclerosis (MS) affects 400,000 Americans and is the leading nontraumatic cause of neurological disability in young adults. Although MS is progressive, it is not fatal, and patients generally have a normal lifespan. However, progressive disability imposes increasing limitations and reduced quality of life for these patients. Newly released consensus guidelines offer neurologists and primary care physicians direction to improve the differential diagnosis and develop strategies to facilitate early and accurate diagnosis of MS. A number of factors must be considered when selecting a treatment regimen for patients with MS, including variations in clinical and MRI evidence of disease. The discovery and broad application of MRI in medicine has led to an increased awareness of the number of patients with incidental white matter pathology in the CNS. The natural history or evolution of such individuals with respect to their risk of developing MS is unclear, but a need for further studies on this subject and physician awareness is essential for progression of disease therapy in MS. Multiple sclerosis (MS) affects 400,000 Americans and is the leading nontraumatic cause of neurological disability in young adults. Although MS is progressive, it is not fatal, and patients generally have a normal lifespan. However, progressive disability imposes increasing limitations and reduced quality of life for these patients. Newly released consensus guidelines offer neurologists and primary care physicians direction to improve the differential diagnosis and develop strategies to facilitate early and accurate diagnosis of MS. A number of factors must be considered when selecting a treatment regimen for patients with MS, including variations in clinical and MRI evidence of disease. The discovery and broad application of MRI in medicine has led to an increased awareness of the number of patients with incidental white matter pathology in the CNS. The natural history or evolution of such individuals with respect to their risk of developing MS is unclear, but a need for further studies on this subject and physician awareness is essential for progression of disease therapy in MS. Does having a seizure mean epilepsy, or is it an isolated neurological event? Failure to recognize diagnostic uncertainty between the epilepsies and non-epileptic events may be a factor in high rates of misdiagnosis. Many patients who have been misdiagnosed as having epilepsy have had previous EEGs interpreted as epileptiform that contributed to the misdiagnosis of epilepsy. Patients are often faced with the need to tolerate complex cocktails of medications over long periods of time causing side effects of these chronic medications to be of concern for both physicians and their patients. Pharmacologic management of patients with epilepsy is one of the greatest unmet needs of the disease. It is important that clinicians are aware of the evidence for current and emerging therapies so they can individualize care of their patients. There is no real "Gold Standard" of guidelines for improving diagnosis and treatment of epilepsy, but clinicians need to be aware of the most recent guidelines and how they can incorporate them in their practice. In this neuroscienceCME webcast, the experts will explore these clinical challenges of managing patients with epilepsy, and will provide insights and strategies for the improvement of patient care.

Schizophrenia is notable for its substantial impact on everyday functioning, being one of the top five causes of disability in individuals under the age of 25. In most cases, individuals who develop schizophrenia manifest behavioral changes prior to the formal diagnosis of the condition. These changes, referred to as the "prodrome," are variable across individuals who may eventually develop schizophrenia. Proactive risk assessment in other chronic diseases such as cardiovascular disease, diabetes mellitus, and cancer has brought about advances in early detection and intervention leading to substantial reductions in morbidity, mortality, and improved quality of life. It is imperative that this type of proactive intervention becomes part of timely diagnosis and better outcomes for patients with schizophrenia. Clinicians face challenges designing treatment plans for patients with schizophrenia that will maximize adherence and reduce side effects. Relapse rates tend to be quite predictable, can be affected by adherence, and have multiple adverse consequences. Minimizing relapse requires a coordinated effort between the patient, provider, family, and multidisciplinary treatment team. In this interactive neuroscienceCME activity, faculty will focus on the impact of treatment selection, phase of illness, medication adherence, and side effect burden on the journey toward remission and recovery in schizophrenia. This activity also includes a special "After the Show" segment during which the faculty answers additional audience questions in an informal Q&A session.

Attention-deficit hyperactivity disorder (ADHD) affects 8% of school age children. It presents with symptoms of inattention, hyperactivity/impulsivity, or both. Currently, three subtypes of ADHD are defined: ADHD inattentive (ADHD/I), ADHD hyperactive/impulsive (ADHD/H) and ADHD combined (ADHD/C). However, in the last decade heated debate has emerged that questions how this disorder should be conceptualized, defined, and categorized. Key questions have been: "Is it a category or a continuum?" and "How do comorbidities relate to currently defined subtypes?" In these interactive, evidence-based neuroscienceCME Journal Club sessions, the faculty will explore new data that may re-focus how we view ADHD subtypes. Such exploration will allow clinicians to anticipate how developers of the DSM-V might revise ADHD diagnostic criteria. Optimal management of bipolar disorder begins with accurate diagnosis. The Depression and Bipolar Alliance reported that the misdiagnosis rate is 67% for bipolar patients with the most common misdiagnoses being unipolar depression (60%), anxiety disorder (26%), schizophrenia (18%), and borderline or antisocial personality disorder (17%). When diagnosis is inaccurate or delayed, initial therapy is not properly derived or promptly delivered.
