To capture the oral history about these and later developments in Australia, Brian Draper interviewed many of the key individuals who were instrumental in the changes that occurred. Attention-deficit hyperactivity disorder (ADHD) affects 8% of school age children. It presents with symptoms of inattention, hyperactivity/impulsivity, or both. Currently, three subtypes of ADHD are defined: ADHD inattentive (ADHD/I), ADHD hyperactive/impulsive (ADHD/H) and ADHD combined (ADHD/C). However, in the last decade heated debate has emerged that questions how this disorder should be conceptualized, defined, and categorized. Key questions have been: Is it a category or a continuum? and How do comorbidities relate to currently defined subtypes? Schizophrenia is notable for its substantial impact on everyday functioning, being one of the top five causes of disability in individuals under the age of 25. In most cases, individuals who develop schizophrenia manifest behavioral changes prior to the formal diagnosis of the condition. These changes, referred to as the "prodrome," are variable across individuals who may eventually develop schizophrenia. Proactive risk assessment in other chronic diseases such as cardiovascular disease, diabetes mellitus, and cancer has brought about advances in early detection and intervention leading to substantial reductions in morbidity, mortality, and improved quality of life. It is imperative that this type of proactive intervention becomes part of timely diagnosis and better outcomes for patients with schizophrenia. Clinicians face challenges designing treatment plans for patients with schizophrenia that will maximize adherence and reduce side effects. Relapse rates tend to be quite predictable, can be affected by adherence, and have multiple adverse consequences. Minimizing relapse requires a coordinated effort between the patient, provider, family, and multidisciplinary treatment team. In this interactive neuroscienceCME activity, faculty will focus on the impact of treatment selection, phase of illness, medication adherence, and side effect burden on the journey toward remission and recovery in schizophrenia. This activity also includes a special "After the Show" segment during which the faculty answers additional audience questions in an informal Q&A session.

Optimal management of bipolar disorder begins with accurate diagnosis. The Depression and Bipolar Alliance reported that the misdiagnosis rate is 67% for bipolar patients with the most common misdiagnoses being unipolar depression (60%), anxiety disorder (26%), schizophrenia (18%), and borderline or antisocial personality disorder (17%). When diagnosis is inaccurate or delayed, initial therapy is not properly derived or promptly delivered. Optimal management of bipolar disorder begins with accurate diagnosis. The Depression and Bipolar Alliance reported that the misdiagnosis rate is 67% for bipolar patients with the most common misdiagnoses being unipolar depression (60%), anxiety disorder (26%), schizophrenia (18%), and borderline or antisocial personality disorder (17%). When diagnosis is inaccurate or delayed, initial therapy is not properly derived or promptly delivered. Pain is a common complaint in the primary care setting, and the standard of practice is to assess for the cause of the pain. Patients often present with painful symptoms of unknown etiology that cannot be defined by the degree of inflammation or damage to muscle tissue. Fibromyalgia affects an estimated 10 million people in the United States and is especially common in women. Many patients have additional symptoms including fatigue, headaches, irritable bowel syndrome, cognitive problems/memory lapses, sleep disturbances, anxiety, and depression. A recent CME Outfitters survey found that 62.8% of respondents were not confident or only somewhat confident in their ability to diagnose and manage fibromyalgia. In this neuroscienceCME Clinical Chart Review, the faculty will take participants through a patient case to demonstrate the complexities of appropriate assessment, diagnosis, and management of fibromyalgia, and provide insights and strategies for improving patient care and long-term outcomes.

According to the U.S. Census Bureau, more than 60 million people live in rural areas and it has been well documented that these Americans face specific challenges to accessing, utilizing, and paying for quality healthcare. Despite 20% of Americans living in rural areas, only 10% of the nation’s physicians practice in rural areas, with access to and availability of mental health specialists, such as psychiatrists, psychologists, psychiatric nurses, and social workers significantly lacking. Integrating mental and medical health in these rural areas has been a focus of many states to provide access and health education to providers and patients. It is critical that best practices implemented throughout the nation are shared among clinicians so that all providers can collaborate and improve their knowledge and performance, gain insights to the latest evidence, and provide optimal care services to patients and their families. Evolving technology has provided new opportunities to provide access to services and address barriers presented to patients and physicians practicing in remote rural communities. As technology improves, costs decrease and access increases—these forces help to create opportunities for increased collaborative care management. I'm Diana Dirkby, and I'm living with paranoid schizophrenia. In this podcast, I'll open up about my experiences with this complex brain disorder while also providing a comprehensive overview of schizophrenia itself. Despite sharing common symptoms, each individual's journey with schizophrenia is unique. We all seek tools and strategies to manage our symptoms within the context of our unique lives.

As mental health consumers, we are responsible for sharing our experiences openly and honestly. By doing so, we can help combat the stigma associated with schizophrenia. We can empower listeners to understand what psychosis truly feels like, dispelling the fear and misconceptions that often surround it. While a schizophrenic episode can be an intense and overwhelming experience, it's important to remember that the person experiencing it is usually not a threat to others.
Beyond my experiences with schizophrenia, I'll also share aspects of my life that transcend my mental health condition. This serves as a reminder that mental health consumers are multifaceted individuals, not defined solely by their diagnoses.

My fiction novel, "The Overlife: A Tale of Schizophrenia," is based on a deeply personal exploration of my own experiences and those of my mother. Join Dr. Scot Bay every Wednesday night at 7PM while he discusses Psychiatry and mental health with guests. Scot Bay, MD is Board Certified in Psychiatry, and has a private practice in Roswell, GA. He majored in Psychology at the University of Rochester and attended medical school at New York Medical College. He completed his residency in Psychiatry at St. Vincent's Hospital in New York City. He was clinical director of an outpatient clinic at St. Vincent's Hospital in Harrison, New York until 1995 when he moved to Columbus, GA, where he was in private practice and was an assistant medical director at the Bradley Center of St. Francis. He moved to the Atlanta area in August of 1998. Dr. Bay specializes in the evaluation and management of mood and anxiety disorders. He has lectured all over the Southeast regarding practical and innovative uses of psychiatric medications, and also has participated in clinical research regarding novel uses for current medications as well as potential new medications. Dr. Bay is concerned with helping America's Web Radio listeners cope better with stress, and feel well emotionally. He also strives to reduce the stigma in the general public regarding mental illness and psychiatric treatment by debunking myths, discussing news stories relating to mental health issues, and giving a balanced explanation of media reports on the latest developments in medical research. Listeners to “Psychiatry Today” will hear useful tips on how to reduce the impact of stress on their lives and how to achieve better emotional well being.
